What is Home Care?

Home Care refers to any type of care that is provided to the client in their home. The type of care includes companionship/homemaking services, personal care services. These services can help maintain or improve your health and can be provided temporarily after a hospital stay, or an ongoing basis if you need continued help.

What areas do you cover?

Heart @ Heart @Home support workers are ready to support you in whatever region you live in Ontario.

Do you have a minimum Service requirement?

We have a minimum 3 hour requirement, however we can be flexible depending on your requirements.

What are your hours?

The office is opened from 8:00 am – 6:00 pm. However, you are able to leave messages after hours that are checked regularly.

Caregivers are available for as little as a few hours a visit up to 24 hours, 7 days a week, 365 days a year.

How is my support worker chosen for me?

During the initial consultation and the in-home visit, the Client Care Manager will document the services the client required. After that, the Client Care Manager carefully selects the best caregiver fit for the client and will arrange an introduction between the client and the caregiver. We strive to ensure that everyone agrees and understands what services are to be provided.

Does the same caregiver work each visit?

We make every effort to always send the same caregiver for each visit. If the service is seven days per week, then, we will generally assign two to four caregivers. We always have backup caregivers if someone is sick or on vacation.

Do you provide services in facilities and hospitals?

Yes! Wherever your care is needed, we are there to lend a helping hand. We provide companionship or personal care to residents at retirement homes, nursing homes and hospitals who may desire additional attention or personalized care.

What is the difference between ADLs &IADLs

Activities of Daily Living (ADLs) include the basic tasks essential for day-to-day functioning, such as bathing, dressing, grooming, eating, mobility and toileting. Many clients, especially seniors require assistance with IADLs rather than ADLs. Some clients for peace of mind, want someone to escort them when they are shopping, running errands or attending Doctor appointments. Other clients may welcome assistance with their bill paying or just keeping in touch with family and friends.

In some cases, intermittent help from a family member or friend may be all that is needed. However, in many cases, particularly when family or friends are unavailable and the importance of scheduling these activities is critical, informal care arrangements may not be adequate.

Heart @ home can provide Personal Care Services to assist our clients with all ADLs.

Instrumental Activities of Daily Living (IADLs) are considered those activities which are less basic than the traditional ADLs. IADLs, nevertheless, are important in enhancing a client's quality of life. IADLs include such activities as errands, paying bills, cleaning, doing the laundry and meal preparation.

Many clients, especially seniors require assistance with IADLs rather than ADLs. Some seniors want someone to escort them when they are shopping and help them avoid situations that might cause them to fall. Other seniors may welcome assistance with their bill paying and medical appointments.

Heart @ home offers an array of companionship and homemaking services to assist our clients with the IADLs.

Do you offer services at night or overnight?

Yes, we offer overnight service. There are two types of shifts and the level of care is dependent upon the client's requirements. The first is a "sleep shift" which offers comfort that we are available if the client wakes and needs assistance. The second choice is that our caregiver is awake and alert at all times. The choice is yours!

How much does in-home care cost?

Pricing varies depending on the services required, number of hours per week, holidays hours, ETC.

How do I know that your Caregivers are trustworthy and that I will be safe?

Our strict and thorough screening process ensures that all of our caregivers meet the highest possible standards for the delivery of personal care services. Each and every member of staff has an up-to-date criminal record check, and are all legal citizens of Canada. We require all our caregivers to be up-to-date with their CPR certification and WHIMIS certificates.

Regular "Supervisory Visits" are made to your home by our Case Managers in order to make absolutely certain that the care you are receiving meets the standards that we, and you, expect. These visits are arranged with you, but remain unknown to your caregivers, who know that they will receive 'surprise' visits from Supervisors and must, therefore, be on their best behaviour and work to the absolute best of their ability at all times. Your safety is our top priority.
